HOW A LIP FLIP WORKS

A Subtle Change That Lets Your Smile Shine

The orbicularis oris is the circular muscle around the mouth. In some smiles, its action pulls the upper lip inward or makes the pink lip show less. A lip flip uses very small doses of a botulinum toxin type A neuromodulator—often Botox® Cosmetic, with product selection determined by the provider—to relax selected fibers near the upper-lip border. As that muscle activity softens, a little more of the existing pink upper lip may become visible.

This is a change in lip presentation and movement, not actual lip augmentation. No filler gel is added, and there is no increase in lip tissue. The resulting difference tends to be modest, especially compared with hyaluronic-acid lip filler.

A lip flip may suit someone whose upper lip tucks inward when smiling. It is generally an off-label cosmetic use of neuromodulator medication. Your provider will discuss the potential benefits and functional trade-offs before treating the delicate muscles around your mouth.

WHAT A LIP FLIP MAY HELP

Define Your Lip. Keep It Subtle.

Lip flips work best when the concern is how the upper lip moves, not simply how much lip volume a patient has. The distinction is key to choosing the right treatment.

More Visible Upper Lip

If the upper lip rolls inward during a smile, selective muscle relaxation may reveal a little more of its natural pink surface without increasing volume.

A Softer Lip Border

For certain faces, a small shift in upper-lip movement can make the lip edge or Cupid’s bow look a touch more pronounced. It will not reshape every lip or correct marked asymmetry.

Select Smile & Lip-Line Concerns

Depending on anatomy, neuromodulators near the upper lip may soften some movement-related vertical lines. A gummy-smile concern sometimes requires a different or additional injection pattern and evaluation.

YOUR CONSULTATION

Is a Lip Flip the Right Kind of Refinement?

  • Your upper lip disappears when smiling: the lip seems to curl under instead of staying visible.
  • You prefer very subtle change: you want to explore definition without injectable filler volume.
  • You understand the limits: a lip flip cannot replace natural lip tissue or dramatically enlarge thin lips.
  • You are comfortable with maintenance: the effect fades and may need to be repeated if desired.
  • You need strong lip movement for work: singers, wind-instrument players, and people whose activities depend on precise oral muscle control should discuss trade-offs.
  • You already experience oral weakness or swallowing problems: a neuromodulator may not be appropriate.
  • You have an infection near the injection site or a contraindication: treatment should not proceed without proper clinical evaluation.
  • You are pregnant or breastfeeding: discuss the limited safety information; elective cosmetic injections are generally deferred.

If your real goal is significantly more fullness, shaping of the vermilion border, or replacement of volume that has diminished over time, explore our lip filler guide or ask about Juvéderm® and Restylane® options.

WHAT TO EXPECT

Small Injections. Careful Decisions.

The procedure itself can be quick, but the planning matters because the lip muscle also helps you speak, drink, and eat.

01

Talk Through Your Goal

Tell us what changes when you smile, whether your upper lip rolls inward, and what look you would like to achieve. Bring photos if they help explain the concern.

02

Evaluate Your Natural Smile

Your provider observes lip movement at rest, during a smile, and while pursing. They review oral function, treatment history, allergies, medications, and possible contraindications.

03

Discuss Filler Versus Neuromodulator

Before injecting, we explain whether muscle relaxation, volume restoration, or no procedure would better match your anatomy. You will hear what a lip flip cannot do.

04

Place Small, Targeted Injections

If a lip flip is appropriate, the provider places a small amount of neuromodulator in carefully chosen points near the upper lip. Dosing and choice of product depend on clinical evaluation.

05

Follow Aftercare & Reassess

Follow the practice’s guidance for the first day and avoid pressing or massaging injection sites. Changes appear gradually; contact the practice if mouth movements feel unusually impaired.

The mouth is a high-movement area. For that reason, conservative dosing and realistic expectations are especially important.

UNDERSTANDING THE OPTIONS

Lip Flip or Lip Filler? Know the Difference.

These procedures solve different problems. One changes movement; the other changes volume. A natural-looking result starts with choosing the correct tool—not the most popular one.

Neuromodulator Lip Flip

Targets a small portion of upper-lip muscle activity to reveal more of the existing upper lip. It is a subtle, temporary, typically off-label technique that does not add volume.

Hyaluronic-Acid Lip Filler

Injects filler gel to add or restore volume, shape, and sometimes structural definition. It can make a greater visible difference but introduces its own risks, including rare vascular complications.

A Carefully Combined Plan

Some patients may benefit from both movement adjustment and volume enhancement. A combined approach is not automatically better; your provider will prioritize balance and avoid overtreatment.

TIMELINE & AFTERCARE

A Little Adjustment. Not an Overnight Transformation.

An immediate dramatic pout is not the expected outcome. A neuromodulator lip flip can begin taking effect over a few days, with the appearance more settled after roughly one to two weeks. There may be early injection-site swelling or bruising that should not be confused with the final effect.

Results typically last about six to twelve weeks in many patients, although this varies. Frequent movement around the mouth can make the change shorter-lived than neuromodulator treatment in other facial areas. If you love the effect, ask about appropriate timing for maintenance rather than assuming it lasts as long as a forehead treatment.

Because the goal is a tiny difference in appearance, comparison photos taken with the same smile and lighting can be more informative than watching for a dramatic change.

What to Watch For

  • Mild soreness, swelling, or a small bruise around injection sites can occur.
  • Temporary trouble pursing the lips, drinking through a straw, or whistling is possible.
  • Some people notice changes when pronouncing certain sounds or keeping liquid in their mouth.
  • Unintended asymmetry, drooling, or an altered smile should be discussed with your provider.
  • Urgent care is necessary for unusual swallowing or breathing difficulty after botulinum toxin treatment.
SAFETY & EXPECTATIONS

The Smallest Treatments Deserve Careful Planning

A lip flip uses a prescription neuromodulator in an area essential for everyday tasks. Treating too much of the orbicularis oris or placing injections inappropriately can affect smiling, sipping, speech, eating, or maintaining a seal around a straw. These effects are usually temporary but can be bothersome while the treatment is active.

Neuromodulators are not approved by the FDA specifically for the cosmetic lip flip procedure. Your provider should explain the off-label nature of treatment, give informed consent information, and review relevant medical conditions, previous toxin treatments, and current medications.

When Another Option May Be Better

  • Thin lips from lost volume: consider appropriately selected lip filler.
  • Deep vertical wrinkles at rest: consider skin-quality treatments in addition to or instead of neuromodulators.
  • Marked lip asymmetry: a lip flip might not correct the cause.
  • Speech, swallowing, or muscle concerns: a lip flip may not be an appropriate elective choice.
  • Seeking permanent change: lip flip results fade over time.

COMMON QUESTIONS

Lip Flip FAQs

What is a lip flip?

A lip flip uses carefully placed, small neuromodulator injections to temporarily relax portions of the muscle around the upper lip. For some people, a little more of the pink upper lip becomes visible, especially when smiling. It does not inject volume into the lips.

Is a lip flip the same as lip filler?

No. A lip flip uses a neuromodulator such as Botox to modify upper-lip muscle movement. Lip filler usually uses hyaluronic acid gel to add volume, shape, or definition. A consultation determines whether either or both approaches fit the goal.

Does a lip flip make lips bigger?

A lip flip does not add lip tissue or volume. It can subtly change how much of the existing upper lip shows, which may make the upper lip appear slightly fuller in some expressions.

How long does a neuromodulator lip flip last?

Effects are temporary and often last roughly six to twelve weeks, although timing varies by product, dose, facial movement, and individual response. A lip flip may wear off sooner than wrinkle-relaxer treatment elsewhere on the face.

When will I see a difference after a lip flip?

The effect develops over several days and may take about one to two weeks to fully settle. Avoid judging the final look immediately after treatment.

Can a lip flip help when my upper lip disappears when I smile?

It may help selected patients whose upper lip rolls inward with a smile by relaxing part of the orbicularis oris muscle. The result is subtle and depends on your natural lip anatomy and smile pattern.

Will I be able to drink from a straw or pronounce words normally?

Most people continue daily activities, but relaxing the lip muscle can temporarily affect straw use, whistling, spitting, speech sounds, sipping, or keeping liquids in the mouth. Discuss these trade-offs carefully with your provider.

Is a lip flip FDA approved?

Using neuromodulator injections for a cosmetic lip flip is generally an off-label application. Although products such as Botox have FDA-approved indications in other treatment areas, those approvals do not mean a lip flip itself is an FDA-approved indication.

Can I combine a lip flip and lip filler?

Sometimes. A lip flip changes muscle movement, while filler adds volume and definition. A combined plan may fit certain goals, but a conservative assessment is important to avoid unnecessary treatment.

What if I want more noticeable lip volume?

If the primary goal is added size, contour, or correction of volume loss, hyaluronic-acid lip filler may be more appropriate than a lip flip. The provider will discuss both approaches and their different risks.

What are the risks of a lip flip?

Possible risks include temporary bruising, swelling, tenderness, asymmetry, altered smiling, drooling, and difficulty with certain mouth movements. All botulinum toxin products also carry serious warnings about possible distant spread of toxin effects.
